Pay Rate: Range $2600 to $2800 Weekly Gross pay based on a 40\-hour work week (Includes non\-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments
Unit: Home Health Hospice
Shifts available: Days: 8 Hours;
Shift timings: Days, 8a â 5p
Schedule: 40 hours / week guaranteed, 5 shifts per week, Mon\-Fri, 30 points weekly productivity expectation
Position Requirements:
- Registered Nurse: 1 year \- Required
- Recent/Current RN Home Health experience: 1 year within the last 3 years â Required
- Home Care Home Base (HCHB) charting \- Required
- OASIS experience \- Required
- Must have valid Driverâs License and Personal Auto Insurance
Education, License and Certification Requirements:
- ASN or an equivalent â Required
- BSN or an equivalent â Highly preferred
- RN License for State of the state applying or Compact if acceptable \- Required
- BLS Certification from AHA or American Red Cross \- Required
Job Description:
- Provide and coordinate care and case management for assigned home health patients in comfort of their homes.
- Develop and implement nursing care plans including SOC, and all types of visits as assigned.
- Performs Case Management, Admissions and all types of visits for the assigned patient case load
- Support patient comfort, pain management and ability to stay at home \- enabling them to spend quality time with their loved ones.
- Empower patients and families by educating them on disease management and pain management.
- Provide Home health nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
- Ensure physiciansâ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
- Ensure patientsâ status is assessed on an ongoing basis and pertinent information is gathered and documented.
- Notify physicians of significant changes in status.
- Document and complete charting for all patient visits within 24 hours.
- Provide bereavement support to patientâs families as needed.
- Territories to cover: Thurston, Lewis, Mason, Pierce counties \- Need would be in any of those counties
