Responsibilities

• Provide skilled nursing assessments and direct patient care in home settings.


• Develop, implement, and update individualized patient care plans.


• Monitor patient progress and document all nursing interventions and patient responses accurately.


• Collaborate with physicians, therapists, and other healthcare professionals to ensure holistic patient care.


• Educate patients and their families on health management and disease prevention.


• Adhere to all company policies, state regulations, and patient confidentiality guidelines.

Requirements

• Active Registered Nurse (RN) license in the state of Michigan.


• Experience in home health nursing or a related nursing field preferred.


• Ability to work independently and manage time effectively across multiple patient visits.


• Strong communication and interpersonal skills.


• Reliable transportation and valid driver’s license.


• CPR/BLS certification.
