Responsibilities

• Provide professional nursing care to patients in a home health setting, following physicians’ orders and guidelines.


• Assess patient health status and develop individualized care plans.


• Monitor patient progress and communicate effectively with patients, families, and healthcare team members.


• Administer medications, treatments, and therapies as prescribed.


• Document patient visits accurately and timely, ensuring compliance with company and regulatory standards.


• Educate patients and families on disease management, medication administration, and lifestyle modifications.


• Collaborate with interdisciplinary teams to promote quality patient outcomes.

Requirements

• Current and valid Registered Nurse (RN) license in the state of Michigan.


• Experience in home health nursing or related healthcare field preferred.


• Strong clinical assessment and critical thinking skills.


• Excellent communication and interpersonal abilities.


• Ability to work independently and manage a flexible schedule.


• Reliable transportation to travel within Dearborn Heights and surrounding areas.
