Job Description - Care manager for older adults

We are looking for a social worker or nurse based in New York, passionate about the older adult population, and looking for flexible remote work.

We are hiring Hera Heroes (care managers) on a remote 1099 basis to help older adults navigate and manage aging at home. Heroes that work 35 hours or more / week will be offered insurance.

What we are looking for:

MSW or nursing degree, clinical license not required but nice to have

2+ years of experience in care management

Passionate and experienced in serving our aging population, relentless in delighting families

Excited about innovating a space that hasn’t changed in far too long

Concise, articulate communication

Job Type: Contract

Pay: $40.00 - $60.00 per hour

Expected hours: 15 – 40 per week
