Agape Health and Wellness, part of the Transformation House continuum of care is adding Contract MHP/MHP Licensing Candidates.  MHP/Mental Health Professional : LPCC, LPC, LICSW, LMFT, or Licensing Candidate

🧠 Your Role: Empowering Recovery Through Clinical Excellence

As a Mental Health Professional (MHP), you’ll play a key role in guiding adults toward mental wellness and lasting recovery. This position offers the chance to provide meaningful, person-centered care while working within a supportive and collaborative team.

What You’ll Do:

• Conduct comprehensive diagnostic assessments to inform individualized treatment plans


• Provide individual, group, and family therapy with a focus on co-occurring disorders


• Facilitate psychoeducational and therapeutic groups designed for dual diagnoses


• Collaborate with a multidisciplinary team to develop holistic care strategies


• Support and mentor clinical staff on best practices in dual-diagnosis treatment


• Identify and address barriers to mental health and recovery using evidence-based approaches

You’re both empathetic and skilled — able to connect deeply while applying strong clinical judgment. You thrive in a collaborative environment and are motivated by helping others grow.

Qualifications:

• Licensed or license-eligible in Minnesota as an LICSW, LPCC, LMFT, or Licensed Psychologist


• Familiarity with MN Statutes 245G, 245A, 626.556, 626.557, and 626.5572


• Ability to pass a DHS background study


• Comfortable with technology and basic computer use
