Position Overview
The Substance Use Disorder (SUD) Case Manager provides comprehensive, recovery-oriented case management services to individuals with substance use disorders, including those with co-occurring mental health conditions.
This role focuses on care coordination, resource linkage, advocacy, and ongoing support to promote treatment engagement, stability, and long-term recovery. Services are delivered in accordance with agency policies, Medicaid requirements, and applicable state and accreditation standards.
Essential Duties and Responsibilities
Complete initial and ongoing assessments related to substance use, recovery needs, and
social determinants of health.
Collaborate with the interdisciplinary treatment team to develop and update
individualized service plans.
Coordinate services with therapists, medical providers, recovery residences, courts,
probation/parole, and community partners.
Link clients to housing, employment, transportation, benefits, medical care, and
recovery support.
Provide recovery-oriented, person-centered case management services.
Complete timely, accurate, and compliant documentation in the electronic health record.
Monitor client safety, relapse risk, and psychosocial stressors.
Participate in supervision, team meetings, and required training.
Qualifications
Associate’s or Bachelor’s degree
Experience working with individuals with substance use disorders or co-occurring
disorders preferred.
Active CDCA Certification Required, LCDC II-III, or comparable credential preferred.
Strong communication, organization, and time-management skills.
Work Environment
Telehealth, home office-based, and/or clinical setting.
Local travel may be required, Community-based as needed.
Ability to manage caseload expectations and documentation timelines.
