Requirements:
• Graduate of an accredited nursing program; BSN preferred
• Active RN license in the state of Connecticut
• Minimum 1 year of acute care or equivalent experience preferred
• Home Health experience
• Valid driver’s license with insured, reliable vehicle and clean driving record
• RN must be comfortable with regular travel across all three counties
• Strong communication and interpersonal skills
Responsibilities:
• Provides skilled nursing care in accordance with the State Nurse Practice Act and home health standards
• Completes comprehensive assessments, including OASIS, within 48 hours of referral or physician order
• Develops and follows individualized Plans of Care based on clinical data and best practices
• Submits timely documentation: Plan of Care and supplemental orders within 48 hours
• Delivers skilled interventions aimed at achieving client goals within defined timeframes
• Updates care plans based on client progress and communicates changes promptly
• Demonstrates competency in assessments, teaching, and medication administration
• Respects cultural, individual, and spiritual differences in care delivery
• Communicates effectively with physicians, team members, and clients
• Maintains accurate, timely, and legible documentation per company guidelines
• Actively participates in team meetings and clarifies reporting responsibilities
• Takes independent action to ensure quality care and refers to other disciplines as needed
• Maintains client confidentiality and adheres to HIPAA guidelines
• Follows infection control and safety protocols; reports and prevents hazards
• Uses equipment safely and requests additional training when needed
• Engages in ongoing professional development and mandatory education
• Supervises home care team members per company policy
• Performs other duties as assigned
• Physically able to lift 20–50 lbs, climb stairs, and work in various home settings
• Must travel daily to client homes and professional appointments using phone, computer, and clinical tools
