Responsibilities
- Administer prescribed medications and treatments according to orders and facility policy
- Monitor residents’ conditions, perform focused assessments, document findings, and escalate changes to the supervising RN or physician
- Provide direct patient care, including activities of daily living, baths, positioning, and mobility assistance
- Perform routine nursing procedures (vital signs, wound care, specimen collection) and accurately chart results
- Collaborate with the interdisciplinary team to update care plans and ensure continuity of care and safety
- Maintain infection prevention protocols, medication safety practices, and patient privacy in accordance with regulatory standards
- Communicate effectively with residents, families, and team members to support quality outcomes and resident comfort

Requirements
- Current LPN license (active, in good standing)
- Ability to work 3x12-hour night shifts as scheduled
- Experience in a long-term care or skilled nursing environment preferred
- Strong communication, documentation, and critical-thinking skills
- Willingness to participate in a travel assignment and adapt to a new team and setting
