Responsibilities
- Perform comprehensive OT evaluations for acute-care inpatients and document findings to guide care planning.
- Develop and implement individualized OT treatment plans addressing activities of daily living, mobility, fine motor skills, cognition, and safety.
- Deliver evidence-based OT interventions, including ADL training, energy conservation strategies, hand therapy considerations, and gait/mobility training as appropriate.
- Collaborate with an interprofessional team (physicians, nurses, therapists, social work, case management) to support patient progress and timely discharge planning.
- Monitor patient progress, adjust goals, and document outcomes in the medical record in alignment with regulatory and facility policies.
- Educate patients and families on home/activity programs and safety strategies to support transitions and optimize functional independence.

Requirements
- Active Occupational Therapy license in all applicable states of practice.
- Master’s degree in Occupational Therapy or Doctor of Occupational Therapy (OTD) required.
- American Heart Association BLS certification required.
- Valid driver’s license and must be at least 21 years of age.
- Home Health experience preferred but not required.
- Ability to work effectively in a hospital-based setting and participate in a travel assignment.
