Responsibilities
- Perform initial OT assessments in patients’ homes and develop individualized treatment plans to improve activities of daily living and functional skills.
- Deliver OT interventions, including therapeutic activities and adaptive equipment training, to promote independence and safety in daily tasks.
- Complete timely documentation for home health episodes, including evaluations, re-evaluations, discharges, and OASIS-related tasks as applicable.
- Educate patients and family members on activity adaptation, safety strategies, and care techniques to support ongoing functional gains.
- Coordinate with physicians, supervisors, and care teams to schedule visits, adjust plans of care, and ensure seamless transitions across services.
- Maintain compliance with regulatory requirements and credentialing standards; ensure accurate maintenance of patient records and privacy.

Requirements
- Active California Occupational Therapist license
- Basic Life Support (BLS) certification
- Valid driver’s license, auto insurance, and auto registration
- TB test (annual) and immunizations: MMR, Varicella, Hepatitis B, Tdap (10 years), Flu (annual)
- Ability to pass drug screening and physical examination
- Willingness to work in a home health setting and to travel as required
- New graduates welcome
