Your responsibilities will include comprehensive patient assessment, development and implementation of individualized treatment plans, and the delivery of therapeutic interventions such as gait and balance training, manual therapy, neuro-rehabilitation strategies, and functional mobility training. You’ll monitor progress, adjust plans based on objective outcomes, document care in electronic health records, and educate patients and families to support home programs after discharge. Collaboration with physicians, therapists, and care coordinators will ensure seamless transitions and optimized discharge planning, while maintaining patient safety and quality standards.
