Responsibilities
- Conduct clinical assessments to identify patient needs and determine appropriateness of physical therapy interventions across age groups.
- Document, prepare/review and complete a Plan of Care (POC), progress notes, phone orders and other clinical records; revise the POC based on evaluation data; collaborate with physicians and the interdisciplinary team.
- Plan and provide treatment for neuromuscular, neurological and orthopedic disorders in line with medical referrals and therapy assessments; monitor patient response and adjust plans as needed.
- Assist/instruct patients and families in transfer techniques appropriate to patient abilities; educate on the use of durable medical equipment ( walkers, wheelchairs, canes, braces, prosthetic devices) and order/adapt equipment to meet home limitations.
- Provide patient/family education regarding therapeutic procedures, considering age, condition, culture and learning readiness.
- Perform and oversee other team members in patient care when applicable; apply infection control and universal precautions; accept clinical assignments within education and competency.

Requirements
- Active Physical Therapist license in the state of Arizona
- CPR certification (current)
- Current driver's license with reliable transportation and auto insurance
- 1–2 years of experience as a Physical Therapist in an acute care or rehabilitation setting
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association-approved program
- Previous home care experience is preferred
