Job Description - Travel Physical Therapist Job

Overview
Physical Therapist | Outpatient focus | Glenwood Springs, Colorado | Start 05/12/2026 | 13 weeks | $2,187–$2,259 per week | 38 guaranteed hours | Shift: 4x10-hour rotating days (includes Saturdays) | Travel assignment in a hospital setting. Healthforce Healthcare LLC is the hiring organization.

Responsibilities
- Evaluate patients in outpatient physical therapy and develop individualized treatment plans
- Implement interventions to improve strength, range of motion, balance, safety, and functional mobility
- Educate patients and caregivers on home exercise programs and self-management strategies
- Document patient progress, communicate with the care team, and adjust plans as needed
- Maintain patient safety, adhere to clinical guidelines, and ensure accurate record-keeping
- Provide coverage as scheduled, including Saturday sessions, to support access and caseload management

Requirements
- Bachelor's degree in Physical Therapy
- Current Colorado PT licensure (DORA) or Compact for Colorado
- BLS through the American Heart Association
- Minimum 2 years of PT experience; 3–5 years preferred
- Ability to work in an outpatient/hospital setting
- Radius rule: applicant’s permanent address must be at least 75 miles away from the assignment facility
