Responsibilities
- Conduct initial evaluations of patients, develop and implement individualized physical therapy plans of care in the home setting.
- Deliver evidence-based interventions to improve mobility, strength, balance, gait, and pain management; progress patients and adjust plans as needed.
- Reassess patient progress, document outcomes, and complete required home health documentation, including OASIS-related tasks as applicable.
- Educate patients and caregivers on home exercise programs, safety, transfers, and fall prevention; provide caregiver training when appropriate.
- Collaborate with case managers, physicians, and other clinicians to coordinate care, ensure patient safety, and plan discharge when appropriate.
- Maintain compliance with clinical standards, infection control, privacy regulations, and agency policies; manage scheduling to meet assignment requirements.

Requirements
- Current California Physical Therapist license
- BLS certification
- Valid driver's license, auto insurance, and vehicle registration
- Immunizations and screenings: TB test, MMR, Varicella, HepB, Tdap (within 10 years), annual influenza vaccination
- Ability to work in a home healthcare environment and travel within the assigned area; willingness to cover weekends if required
- Drug screen and medical exam as part of onboarding
