Job Description - Travel Physical Therapist Job

Overview
Physical Therapist – Long-Term Care / Nursing Home setting in Siler City, North Carolina. This travel assignment starts on 04/27/2026 and runs for 13 weeks with guaranteed 35 hours per week. Responsibilities
- Assess residents’ physical therapy needs and develop individualized treatment plans.
- Implement, monitor, and adjust therapy interventions to improve mobility, strength, balance, and functional independence.
- Reassess progress regularly; document patient status, treatment goals, and outcomes in charting.
- Collaborate with an interdisciplinary team to coordinate care plans and discharge planning.
- Educate residents and family members on exercises, safety strategies, and home programs; ensure patient and family understanding of plan of care.
- Maintain safe, compliant clinical practice and adhere to all relevant policies and infection control guidelines.

Requirements
- Active Physical Therapist license in North Carolina or eligible for licensure.
- Current CPR/BLS certification.
- Ability to practice in a long-term care setting and work with older adults.
- Strong communication, documentation, and teamwork skills.
- Availability to work 35 guaranteed hours per week on a 5x8 day schedule (08:00–16:30).
