Position Requirements:

• Current Physical Therapist experience: 2+ years - Required
  


• Home Health experience - Required
  


• Prior travel experience: Highly preferred
  

Education, License and Certification Requirements:

• PT License for State of Illinois - Required
  


• BLS Certification from AHA  - Required
  

Job Description:

• Provide all aspects of Physical Therapy Services to patients in a home health setting.
  


• Plan and execute rehabilitation programs to help patients return to a higher level of physical function.
  


• Utilize a variety of skills including, but not limited to, therapeutic exercise, neuromuscular re-education, physical modalities, gait training, and manual techniques.
  


• Demonstrate effective communication and strong interpersonal skills to motivate patients and provide compassionate care.
  


• Fleet car will be provided for the therapist to drive.
