Responsibilities
- Deliver physical therapy assistant services to home health patients under the direction of a licensed physical therapist, implementing and adjusting the plan of care as directed
- Administer therapeutic exercises, gait training, balance activities, and ADL interventions; monitor patient response and progress
- Perform modalities and assistive interventions within scope of practice; maintain patient safety and infection control standards
- Document patient encounters accurately and timely; communicate patient status and progress to supervising PT and care team
- Coordinate with caregivers/family members; arrange equipment or assistive devices as needed; participate in care planning and daily scheduling
- Travel within assigned counties (Area #1 and Area #2); weekend coverage may be required and occasional activity outside the service area; comply with productivity and scheduling expectations if applicable

Requirements
- Active California PTA license
- BLS (Basic Life Support)
- Valid driver’s license; auto insurance and vehicle registration
- TB (annual), MMR, Varicella, HepB, Tdap (within 10 years); annual Flu vaccination
- Drug screen and physical clearance
- Minimum 1 year of professional PTA experience; home health experience preferred
- Eligibility to work in the home health setting and to travel as required
- Start date and end date must align with assignment windows; ability to meet travel and scheduling requirements
