Responsibilities
- Perform utilization review for inpatient admissions, including admission/discharge decisions and DRG/coding awareness to ensure appropriate level of care.
- Lead care coordination and discharge planning in collaboration with the interdisciplinary team (IV, respiratory, pharmacy, social services, case management, and unit support).
- Assume charge/lead responsibilities on the unit as needed and participate in ongoing process improvement related to patient flow.
- Document all case management and utilization activities in Epic, maintaining compliance with HIPAA, CMS, and Joint Commission standards.
- Float to like units as needed within scope, including units serving neonates through geriatrics, to support patient care and efficient throughput.
- Communicate effectively with physicians, families, and care teams to facilitate safe transitions and timely discharge planning.
Requirements
- Active WA RN license or Compact RN license; BLS certification required.
- Minimum 2 years RN experience preferred; acute care inpatient experience within the last 3 years required.
- Previous charge/lead experience required; first-time travelers accepted if they have strong, recent acute inpatient experience.
- Inpatient acute care hospital experience is a hard requirement; behavioral hospital experience is not acceptable.
- Units served include ICU, PCU/SDU/IMC, ED, Family Birth, and Adult Medical; must be comfortable working in a rural hospital setting with limited resources.
- On-site location restrictions: locals accepted – no; 50-mile local radius applies.
