Role specifics and benefits:
As a Home Health Registered Nurse, you’ll deliver comprehensive patient care in home settings, collaborating with families and the broader care team to create and implement individualized care plans. Your responsibilities include thorough initial assessments and ongoing monitoring of vital signs and symptoms; wound care and infection prevention; IV therapies and medication management; chronic disease management and education for patients and caregivers; fall prevention and safety optimization in the home; palliative and comfort-focused care when appropriate; timely documentation and communication with physicians, case managers, and social workers; and escalation of changes in patient status to ensure prompt, quality interventions. You’ll coordinate services with physical therapy, occupational therapy, and other disciplines to support functional goals, while empowering patients to participate actively in their care. Documentation is precise, compliant, and timely to support accurate continuity of care and reimbursement. Your role will also involve travel between patient homes, which is supported by our 24/7 clinical and operational teams so you never navigate complex days alone.
This position offers competitive compensation designed to recognize your expertise and commitment. The weekly pay range is $2,039 to $2,116, with guaranteed 40 hours per week, supporting financial stability as you plan your schedule.
