Role Specifics and Benefits
Position overview: A contract-based LTC RN role designed to maintain continuity of care for residents in a long-term care setting, with a start on 02/09/2026. Assignment length is measured in weeks, with extension opportunities based on unit needs and performance. You will be the primary nursing resource for daily resident care, coordinating with the interdisciplinary team to deliver comprehensive, compassionate services.

Responsibilities:
- Deliver direct nursing care to LTC residents, including routine assessments, medication administration, IV management as appropriate, wound care, and monitoring of vital signs and health status.
- Develop, implement, and adjust individualized care plans in collaboration with physicians, therapists, social workers, and families; document changes accurately and timely.
- Lead care coordination for mobility, nutrition, medication reconciliation, infection prevention, fall risk management, pain control, and palliative considerations.
- Provide evidence-based dementia care, behavior support, and de-escalation strategies to maintain resident safety and quality of life.
- Educate residents and families about treatment options, care opportunities, and discharge or transition plans, while upholding residents’ rights and preferences.
- Contribute to quality improvement initiatives, participate in rounds, and mentor less-experienced staff and CNAs to promote best practices.
- Ensure compliance with regulatory standards, facility policies, and safety protocols; maintain meticulous documentation and reporting.
