Responsibilities
- Provide diagnostic, therapeutic and consultative services to patients with communication, cognitive and swallowing disorders.
- Assess, diagnose, and treat speech, language, cognitive, and swallowing impairments; develop and implement individualized treatment plans.
- Document evaluations, treatment plans, progress notes, and discharge planning in accordance with regulatory and payer requirements.
- Collaborate as an integral member of the interdisciplinary care team, coordinating with physicians, nurses, and other therapy disciplines to optimize patient outcomes.
- Adhere to state and federal regulatory license, certification and accrediting requirement guidelines, maintaining compliance and best practice standards.
- Educate patients and families about diagnoses, treatment options, and home exercise programs; participate in ongoing quality improvement activities.
