Sage Group HealthCare Staffing is seeking Physical Therapist \- Home Health/Hospice for a travel / contract position in Littleton, NH.

Pay Rate: $3200 Weekly Gross pay based on a 40\-hr work week (Includes Non\-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage Reimbursement: $0.70/mile for distance covered from office to patient home visits and in between patient home appointments

Shifts available: Days
Shift timings: 8 Hours; Days, 8.00/9.00 am \- 5.00/6.00 pm
Schedule: 40 hours/week guaranteed, 5 shifts per week, Mon\-Fri, No weekends, No holidays, No call
Position Requirements:
- Physical Therapist experience: 2 years \- Required
- Current experience Home Health or Hospice PT experience: 1 year â Required
- Must have experience providing care for patients of all ages
- OASIS experience \- Required
- Must have valid Driverâs License and Personal Auto Insurance
Education, License and Certification Requirements:
- PT License for State of NH or Compact \- Required
- BLS Certification from AHA or American Red Cross \- Required
Job Description:
- Provide all aspects of Physical Therapy services in a home health setting
- Performs examination and evaluation to determine diagnosis and prognosis of patient/client and determine appropriate intervention to gain expected outcomes based on patient/client goals including a collaborative approach with patient/family and health care team.
- Utilizes of appropriate Physical Therapy interventions to produce desired outcomes.
- Utilizes of appropriate documentation and billing to communicate progress and status to physicians, other healthcare professionals and third party reimbursement
- Supervision of Physical Therapist Assistant in accordance to state licensure laws.
- Utilizes critical thinking in making independent judgements and maintains responsibility and accountability for the knowledge of conditions of assigned patients.
- Able to demonstrate the necessary skills and competencies of a physical therapist.
- Keeps work area and equipment clean and orderly by maintaining and practicing hospital and departmental policies and procedures regarding safety and infection control
