The Health Coach will provide personalised support to individuals to empower them take control of their own health and wellbeing. The post holder will facilitate people in establishing and attaining health-promoting goals set by the person in order to change lifestyle-related behaviours, with the intent of reducing health risks, improving self-management of chronic conditions, and improving quality of life The post holder will be responsible for delivering a range of face to face, telephonic and digital health coaching support to individuals from a wide range of settings. Accountability To provide largely a remote coaching service for eligible participants that supports them to gain and use the knowledge, skills and confidence to become active participants in their care and to reach their self-identified health, wellbeing and employment goals. To deliver telephonic and digital health coaching support to a case load of participants, providing personalised psychosocial interventions including, but not limited to, agenda setting, problem solving, motivational interviewing, goal setting and signposting. To understand the boundaries of coaching and ability to identify and act accordingly when coaching is not appropriate and being able to sensitively discuss alternatives or escalate appropriately. To organise and deliver occasional health coaching face to face support, during site based health and wellbeing events To contribute to the monitoring and implementation of all policies and systems as they relate to service delivery, in particular: Health and Safety, Safeguarding, Vulnerable Adults To ensure timely accurate reporting and data collection To take a proactive approach to supervision and training with a demonstrated commitment to personal development Internal • Reporting to Coaching Manager • Supporting the Lead Employability Coach in aspects of health and wellbeing content for Employability Coach interventions • Supporting the wider team of Employability Coaches, in aspects of health and wellbeing sessions This role is home based and providing telephone, digital coaching support with occasional visit to the the London Restart offices to provide face to face and digital support onsite therefore we need applicants to live within commuting distance of London. Qualifications & Experience ESSENTIAL • Relevant health coaching qualification or an accredited health coaching skills programme. • A minimum of six months of direct health coaching delivery • Experience of caseload management • Experience of supporting vulnerable individuals through a change process • Evidence of continuous professional development to ensure the delivery of the most recent and up to date health coaching provision to the correct quality standard. DESIRABLE • Experience in delivering behaviour change interventions for specific lifestyle related issues (Smoking cessation, physical activity, NHS Health Checks, weight management, alcohol reduction) Individual Competencies ESSENTIAL • A strong understanding of the social determinants of health • A strong understanding of population-based approaches including segmentation and risk stratification • A strong understanding of behaviour change principles and methodology. • Demonstrable core skills and competencies as set out in the NHS England 'Health Coaching Implementation and Quality Guide' including but not limited to: • Select and apply a range of health coaching models, conversation frames and techniques • Detailed knowledge of the core concepts and principles of personalisation, patient activation, health behaviour change, self-efficacy, intrinsic motivation and assets-based approaches EEO Statement Maximus is committed to developing, maintaining and supporting a culture of diversity, equity and inclusion throughout the recruitment process.
