MINIMUM QUALIFICATIONS:

  • 1 year of verifiable experience interacting with and effectively communicating with community partners
  • Travel throughout Area 7 (Orange, Osceola, Seminole) is a requirement, and the incumbent must have a valid driver’s license and access to a personal vehicle to perform the job duties of the position in lieu of a county vehicle.
  • 1 year of experience fostering, interacting, and effectively communicating with community partner
  • 1 year of experience in Patient Care Services, HIV testing/counseling, HIV case management and/or disease intervention
  • 1 year of experience in interacting with and effectively communicating clients/patients with a diverse background
  • Emergency Duty

JOB DUTIES AND RESPONSIBILITIES:

The purpose of the Government Operations Consultant I (Early Intervention Specialist) is to conduct intensive outreach with the purpose of linking out-of-care HIV positive clients to medical care. Outreach will be focused on initiating, linking, and sustaining optimal medical care for HIV positive clients. The Government Operations Consultant I will assist with targeted counseling, referral services, linkage to care, and health education and literacy training that enable clients to navigate the HIV system of care.

Engage with community advisory groups to further assist in determining service gaps, reasons for these gaps, and reasons for patients dropping out of care. Work diligently to identify HIV positive clients who are out-of-care and ensure that these clients are assisted with their return-to-care transition. Work with local County Health Departments and Area 7 staff to identify target demographics as well as target geographical service areas.

Provide Ryan White eligibility, HIV counseling, testing and referral services to assist clients with assessing their health risks and HIV status. Ensure linkage to medical care by scheduling doctor’s appointments, ensuring that the client gets to that appointment, and following up on subsequent appointments.  Provide intensive outreach by connecting with service providers and medical professionals to identify out-of-care HIV positive clients and link those clients to medical care. Establish a relationship with clients to ensure successful adherence to their medical care and ensure that clients are retained in care with the assistance and follow up of case management.

Provide health literacy training to HIV positive clients to educate and better equip these clients with the necessary resource tools they may need to help them identify and understand the barriers to care that they may face.

Maintain timely documentation of interactions with clients affected by HIV/AIDS as well as other work-related activities.  Maintain relationships with other resource providers, including agencies, organizations and professionals that provide services for persons affected by HIV/AIDS. Accurately code and document delivered services, education encounters, and other pertinent information in clients’ electronic medical record. Complete and timely submit all administrative documents. Compile and reconcile data and other information into reports, as required.

  • Ability to work independently.
  • Knowledge of and experience with HIV counseling and testing.
  • Knowledge of CDC  HIV/AIDS Prevention Interventions.
  • Ability to work with diverse populations.
  • Ability to maintain client confidentiality.
  • Knowledge of and experience with computers and computer software including but not limited to: Windows (Microsoft Word, Excel and PowerPoint) Outlook, and if applicable: CAREWARE, STARS, Provide, and other reporting software..
  • Knowledge of and experience with methods of data collection, analysis and evaluation. 
  • Ability to utilize problem-solving techniques.
  • Ability to communicate effectively both verbally and in writing.
  • Ability to establish and maintain effective working relationships with others.
  • Ability to work independently, prioritize tasks, plan and organize work, understand and apply rules
