Responsibilities
• Provide case management services to adults incarcerated in Cape County Jail • Assist Jail Navigator with weekly and monthly tasks • Track, review, and follow up with inmates based on Mental Health screening tool • Coordinate transportation for inmates upon release • Provide inmates with appointment times with local mental and physical health providers • Connect inmates to community resources while incarcerated and upon release • Enroll eligible clients in the ERE program for ongoing support and assist with transition to community outreach specialist • Collaborate with jail medical staff(s) and psych provider(s) to support inmates with needs • Establish a line of communication with the Department of Corrections for inmates transferring to prison • Help establish or reestablish mental health services during incarceration and after release
Requirements
• Bachelor's Degree in a Human Services field or equivalent combination of education and experience • Qualifying experience with mental illness, substance abuse disorders, or developmental disabilities • Must have valid driver's license, continuous auto insurance, and reliable transportation
