As Manager Pre Admission Coordinators, you’ll contribute to our success in the following ways:

  • Manages assigned PAC team and performance of designated Skilled Home Health coverage area.
  • Is responsible for the review and quality assurance of referrals (e.g., Review & Approve Referral After Payer Verification) to ensure accuracy of data entry and all documentation is included before approving for Assign LP.
  • Performs follow-up calls or notifies Account Executive (or appropriate party) on pending facility discharge and pending surgery patients and document accordingly.
    • Pre-Admission Coordinator may ask Administrator, or designated Branch Director, for approval to push referral to Assign LP with open items for resolution by Service Line team member(s).
  • Follows up with facility for final discharge orders.
  • Reviews Intake Coordination Notes provided by Intake.
  • Confirms following or certifying physician/provider to ensure they are willing to sign orders.
  • Confirms F2F is scheduled before SOC is performed. Reviews existing F2F to ensure diagnosis matches primary focus of care.
  • Confirms charts within referral paperwork relate to referral patient.
  • Confirms physician demographics to facilitate coordination of order signatures.
    • If information requires update, Pre-Admission Coordinator enters coordination note to trigger workflow, as required.
  • Reviews insurance to identify co-pays, worker’s compensation liabilities, or other issues and communicates issues to Account Executive, or appropriate party, to confirm accuracy and whether this relates to home health services.
  • Requests most recent Health & Physical (H&P) and confirms receipt.
    • If unable to receive timely, Pre-Admission Coordinator will notify CTM (or appropriate Service Line team member) H&P is outstanding.
  • Obtains initial orders for SOC verbally from MD, as needed.
  • Performs Welcome Call to Patient or ensures “Welcome Call” has been performed by Account Executive.
  • Ensures timely resolution of missing information to meet service level and regulatory requirements.
  • Verifies PECOS certification obtained for physicians and resolves issues as needed.
  • Adheres to all applicable county, state, and other regulations.
  • Performs other duties/projects as assigned.

What is Required?

  • Degree from an accredited school of professional nursing.
  • 3+ years of practical experience as a LPN.
  • Current Certified LVN or LPN.
  • Excellent verbal and written communication skills.
  • Proficiency with MS Office Suite (Word, Excel, Outlook)
  • Prior experience with enterprise applications such as Workday and Homecare Homebase is preferred.
  • Must have a dependable vehicle, a valid driver’s license, and current auto insurance in accordance with the laws of the State

You will report to the Vice President Sales Operations.
