Medicare ACO Nurse Practitioner ADMIN - Woonsocket, 25 John A Cummings Way, Woonsocket, Rhode Island, United States of America Req #1526 Monday, May 1, 2023 Thundermist‘s Mission - To improve the health of our patients and communities by delivering exceptional health care, removing barriers to that care, and advancing healthy lifestyles. As a Medicare ACO nurse practitioner, the candidate will provide Medicare wellness visits to a panel of moderate to high-risk Medicare ACO patients with complex chronic health conditions. The Medicare ACO annual wellness visits will be delivered in office-based visits as well as to homebound Medicare ACO patients as well as providing, occasional hospital and emergency room follow-up and diversion visits as needed, and as an extension of the primary care providers care team, within the limits of the training of a nurse practitioner, to patients who are members of Thundermist Medicare ACO. The nurse practitioner is a licensed, autonomous provider executing professional clinical judgment in the care of his/her patients, according to the guidelines, policies and procedures set forth by the agency. The candidate will also coordinate sick visits or additional services as needed for Medicare ACO attributed members. Coordinate efforts with the Care Management Outreach Worker Medicare ACO. Qualifications: Required Qualifications: * RI RN and APRN Licensure * ANA Certification in family medicine or Adult-Geriatric * Ongoing CEU to update skills/therapies. * Proficient in all applicable medical terminology. * Experience with Medicare patients, Medicare Annual Wellness Visit requirements, and Medicare Chronic Care Management requirements. * Experience in community health setting, public health, chronic disease management, community nursing, case management preferred. * Experience working with primary care providers in practice setting to coordinate care and disease management. * Knowledge of disease management, case management, and chronic care principles * CPR/BLS certification Preferred Qualifications: * Certification in one or more of the following: Case Management, Diabetes Education, Asthma/COPD, Cardiovascular * Candidates who do not meet all the preferred qualifications are encouraged to apply. Significant Job Functions: * Patient care - Medicare annual wellness exams, history, physical exam, management of acute and chronic illness for adults with complex chronic health conditions for Medicare ACO patients. * Medicare annual wellness office-based visits and home-based care for patients who are experiencing temporary or long-term physical or behavioral health issues impacting their ability to be seen in an office-based visit. During the visits the practitioner will develop care plans which establish goals, interventions based on risk category and patient‘s identified medical and social needs. Visits will reinforce the importance of staying current on preventative services while identifying potential gaps in care which may result in unnecessary hospitalizations and ER visits. * Work collaboratively with the Care Management Outreach Worker Medicare ACO. * Make recommendations for Medicare ACO members follow-up with their primary care provider. * Follow-up to for transitions of care by performing post-discharge visits in either office based, telehealth, or home visits to reconcile medications, assess members‘ clinical status, and ensure that they have access to all necessary home services. * Participate in performance improvement activities to improve quality of care and safety to assure that the health care team meets and exceeds the established standards of practice and benchmarks. * Maintenance of complete/accurate patient records. * HCC Coding Physical Effort / Environment: * This is a hybrid role. Work may be completed onsite or remotely. * Moderate physical activity, walking, standing, stooping, and occasional lifting. * Potential exposure to blood-borne pathogens, infectious diseases i.e., respiratory (Tuberculosis), and bio- hazardous materials. * Manual dexterity for computer use. * Organization skills, neatness; knowledge of safety and infection control standards of practice. Work Schedule Demands: * Full-time position with a base of 40 hours per week. (31 Clinical Face-to-face hours, 8 administrative hours, 1 hour per week to attend NP Learning Collaborative) Communication Skills: * Interpret technical medical terminology daily.
