Job Description - PACE Center Manager

Position Summary: The PACE Center Manager (RN) is responsible for the day-to-day operations of the day center and the coordination of all care delivered to participants in PACE Program. This includes the management of the Interdisciplinary Care Team as well as the oversight of the facility's operations and systems. Position Duties & Responsibilities:
  • Responsible day-to-day oversight of all components of PACE center, comprised of primary care medical clinics, 24-hour care delivery, preventive services, a provider and healthcare network that encompasses all Medicare and Medicaid services, therapeutic services, PACE centers and alternative PACE centers, home care, transportation, and Quality Assurance and compliance activities.
  • Coordinates the Care Team's care management process to ensure the best care decisions for each participant's changing condition.
  • Facilitates the Interdisciplinary Care Team meetings for development and implementation of care plans.
  • Ensures documentation of team meetings is maintained and changes are recorded in individual care plans. Sends notification to appropriate agencies and regulatory bodies when care plans are changed.
  • Works within the quality management program to evaluate and improve performance.
  • Provides on-call coverage to troubleshoot, advise, teach, coordinate and deliver care and service to participants.
  • Ensures compentent staff perform their functions within the center, and staff and contract providers are properly trained on all policies and procedures.
  • Oversees effective operations of activities, nutrition, personal care, clinical services, rehabilitation and other services of the center within the standards and regulations of the program.
  • In coordination with the Regional Administrator, prepares the annual budget for center operations and monitors budgets by service and per member month.
  • Confirms that participants are oriented to the program and have opportunities to provide feedback about the care and services they receive.
  • Develops, implements, and momtors ongoing compliance with policies and procedures.
  • Coordinates with contract services for meals, transportation and home care; oversees the staff who provide the care management of participants related to these services.
  • Encourages the inclusion of opinions and, when appropriate, representative staff from contract services in the care planning process.
  • Oversees the provision of quality service within expected standards and financial targets.
  • Hires, trains, evaluates and coaches Center staff. Consistently monitors staffing and maintains appropriate levels to provide safe and effective care. Recommends staffing levels and structural changes as needed.
  • Drives Center workforce engagement and positive employee experience. Cultivates a supportive and inclusive work environment for all employees through continuous development, modeling inclusive behaviors, and proactively managing bias. Assists in the resolution of employee relations issues.
  • Facilitates orientation, training, and competency programs for staff to meet regulatory requirements and support performance improvement.
  • Evaluates the equipment and furnishings of the Center, including appropriate maintenance and availability. Makes recommendations for capital equipment; authorizes day-to-day purchase decisions.
  • Collaborates with Quality, Risk, Legal, and Facilities Support Teams. Responsible for compliance with infection control, fire safety and related safety procedures.
  • Manages the contracts for waste management and housekeeping.
  • Ensures durable medical equipment is maintained in safe, usable condition.
  • Coordinates with contract services to ensure understanding of the PACE model and cooperation with the care management process. Coordinates with home health, long term care and medical services according to participant care plans.
  • Collaboratively assists Legal Department in the development of agreements and contracts with provider agencies to ensure comprehensive service availability.
Position Qualifications:
  • Must hold current Registered Nurse licensure in the state of Kentucky or the ability to obtain upon hire.
  • Bachelor's degree in Nursing or health-related field preferred.
  • Minimum one (1) year experience working with the frail and elderly; previous experience with PACE, or in a home care or post-acute healthcare setting preferred. Strong understanding of geriatric patient population, underserved/underrepresented communities and the seriously ill, including the long-term care eligible population.
  • Must possess strong financial and budget management skills and demonstrate ability to build effective teams.
  • Ability to work with minimal supervision, take initiative, and make independent decisions.
  • Strong written and verbal communication skills, as well as the ability to effectively communicate with all levels of the organization and community stakeholders.
  • Excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level of quality and productivity.
  • Strong analytical and problem-solving skills.
  • Food handling permit required, to be completed upon hire.
