Overview

At Dignity Health’s Arizona General Hospital we deliver humankindness through a wide range of health social and support services in the heart of Phoenix Arizona. As part of our team you will provide clinical expertise support and oversight that helps ensure our patients receive exceptional care. Responsibilities

The Therapist provides patient care including evaluating the patient for treatment providing and directing administration of the care plan upon the referral from the physician. Consults with other health care disciplines to coordinate therapeutic programs and discharge planning for individual patients. Instructs patients and caregivers in home program exercises and functional activities. Assists with patient program management while delivering quality patient services.

Qualifications

Minimum:

Internship or rotation in appropriate clinical setting required.

Graduate of an accredited Physical Therapy Program required.

Licensed by AZ State Board of Physical Therapy as a Physical Therapist.

American Heart Association BLS.

Bachelors degree.

Computer skills required.

Preferred:

Prior use of electronic health record preferred.

Masters or Doctorate degree in appropriate clinical area preferred.

2 years progressive experience in clinical area performing patient clinical care preferred.
