As a Chronic Disease Prevention Manager within the NCHD District 5-2 at the Georgia Department of Public Health, you will join a dedicated group of public health professionals that help support the overall mission and vision of protecting lives of Georgian residents in a variety of ways.

Job Responsibilities

The Chronic Disease Prevention Manager promotes community health by assisting individuals and communities to adopt healthy behaviors and by working towards policy, systems, and environmental changes that support healthy eating, active living, and tobacco control/cessation. This position will be based at the North Central Health District office in Macon, GA and does not offer relocation assistance. Candidate must live within a reasonable commuting distance. If relocating to Macon or surrounding area, please specify on the application.

  • Emergency Preparedness and Response: In the event of an emergency requiring activation of Public Health, all public health staff will be expected to perform duties and responsibilities as assigned. This may require work outside the normal work schedule.
JOB RESPONSIBILITIES:

Develop Partnerships and Assess Community Health Needs

  • Develop relationships with staff in public health programs and county health departments to find areas for collaboration to maximize impact of activities and programs.
  • Develop partnerships with and support similar public health programs like SNAP-Ed, Adolescent Health & Youth Development, Cardiometabolic program to control diabetes and hypertension.
  • Engage with community coalitions or community health partnerships to address chronic disease issues. Develop partnerships with community organizations.
  • Leverage North Central Health District’s Community Health Assessment to target priority populations, geographic areas, and/or health behaviors to maximize impact.
Increase Physical Activity and Access to Healthy Food in the Community

  • Complete baseline assessment of access to places for physical activity. Assess gaps in access to opportunities for physical activity.
  • Complete baseline assessment of access to healthy foods in the community (farmers markets, food banks & pantries, nutrition classes/education).
  • Implement a minimum of one evidence-based program or strategy to increase physical activity and/or access to healthier food for a priority population or geographic area.


Tobacco Use Prevention

  • Conduct educational campaign to increase community and tobacco retailers knowledge on Tobacco 21 legislation.
  • Ensure that public health clinical staff are trained on the brief tobacco intervention and make appropriate patient referrals to the Georgia Quit Line.
  • Conduct community-level educational campaign on the dangers of e-cigarettes and emerging tobacco products and cessation resources for youth and young adults.
  • Conduct media campaign on the dangers of e-cigarettes and emerging tobacco products and cessation resources for youth and young adults.
  • Support tobacco-free policy adoption at school districts, colleges & universities, and multi-unit housing communities.
Reporting and Other Administrative Duties

  • Develop annual work plans using provided templates.
  • Participate in required DPH monthly telephone calls, webinars, trainings, collaboration meetings, and technical assistance opportunities.
  • Complete and submit quarterly programmatic progress reports.
  • Submit two success stories annually illustrating one successful Chronic Disease Prevention activity and one Tobacco Use Prevention activity.
SKILLS AND QUALITIES THAT WILL LEAD TO SUCCESS IN THIS ROLE:

  • Self-motivated, energetic.
  • Ability to work as a member of a team, but also able to get work completed individually.
  • Excellent oral, written and interpersonal communications skills.
  • Proficiency with MS Office applications (e.g., Outlook, Word, Excel, PowerPoint)
  • Attention to detail and deadlines in collecting data and completing required reports.

Minimum Qualifications

Bachelor’s degree in education, social, behavioral, or biological sciences or a directly related field from an accredited college or university OR Master’s degree in public health education from an accredited college or university.



PREFERRED QUALIFICATIONS:
First-hand experience working in community health programs with community residents and community partner organizations.

The use of wellness breaks during the workday is authorized to support this philosophy and assist employees in meeting their wellness goals. A maximum of 30 minutes in a given workday may be used to engage in wellness activities, generally in the form of two 15-minute breaks or one 30-minute break.
