We are a medical group practice of outstanding, hand-picked clinicians making more than 1,000 home visits a year in private residences, retirement communities, apartment buildings, and independent and assisted facilities. We are driven to bring care to private residences, retirement communities, apartment buildings and independent and assisted facilities. Our passion is caring for frail, elderly complex patients, – particularly the highest-needs, in their home wherever they reside.
We are looking for a compassionate Primary Care Provider to join our team and provide expertise to our patients in their home. The ideal candidate will have a passion for patient care and excellent communication skills.
This position involves daily house calls.
Work Schedule:
● Monday – Friday, 40 hours per week, 52 weeks of the year (excluding approved time off).
● On-call for nights and weekends.
● Holiday schedules will alternate with other providers (annual rotation of holiday coverage).
● Half-day once a week allocated for administrative, performance improvement, quality review, assurance, and program development rollout. Will collaborate with Chief Medical Officer (CMO) and Practice Administrator on these items.
● Conduct 5-7 home visits (new and established patients) per day.
● Core focus and prioritization of seeing new patients and establishing goals of care/care plans
● Conduct daily morning check-ins
● Lead the 1-hour weekly Interdisciplinary Team (IDT) meeting.
● Provide medical direction, leadership, and education for the office staff and IDT.
● Help to create the core curriculum and teaching materials along with working with the CMO and team members.
● Develop key quality metrics including performance/process/outcome measures.
● Create compliance, safety, risk management, and quality assurance standards/policies/reports.
● Chart review and CII scripts monthly
● Review of NP charts for primary collaboration.
● Perform duties as needed for skilled home health or hospice services that the company may provide.
● Perform services as contracted Medical Director as desired.
● Interview and input candidates to hire, manage performance, etc. any assigned staff.
● Board Certification preferred in the following: Internal Medicine, Geriatrics, Family Practice, General Practice,
Hospice and Palliative care.
● Licensure in the state where employed or a compact party state and be board certified
● CPR Certified
● Must be able to lift a minimum of 50 pounds.
● Valid driver’s license
● Valid and current DEA number with ability to e-scribe when needed.
● Graduate of Accredited Medical School
● Active unencumbered license.
