Pay-rate:  Based on 40 hrs/wk
$70/hr
Shifts : 5x8's - 40hrs per week - 8am-4:30pm

Job Duties:

• Conduct thorough assessments of patients' and families' psychosocial needs, identifying barriers to effective care and facilitating appropriate interventions.
• Develop and implement comprehensive care plans in collaboration with the healthcare team, addressing identified needs and coordinating services to support patient and family well-being.
• Maintain accurate and confidential patient records, documenting all assessments, interventions, and communications in compliance with hospital policies and regulatory guidelines.

Requirements:

• REQUIRES LCSW,
• HI License; At least 1+ year of experience in discharge planning and case management in HOSPITAL setting only
• 2+ year of experience
