Purpose
Cares for residents, clients, and/or members, and fosters the highest degree of quality care under supervision from designated department management and/or other medical staff as appropriate.


15 Craigside:
Health Care Center- Full Time (40 hrs. per week) from 8am 4pm & 4pm to 12:00 midnight (mixed shifts)
Health Care Center- Part Time A (30 to 35 hrs. per week) from 4pm 12am
Health Care Center- Full Time (40 hrs. per week) from 12 midnight to 8am
Health Care Center & Assisted Living combo- Full Time (40 hrs. per week) from 12 midnight to 8am
Health Care Center- Part Time A (30-35 hrs. per week) from 8am to 4pm All shifts require weekday and weekend flexibility. Essential Functions

1. Answers call lights to determine residents’ or clients’ needs (Communities and Day Care only).
2. Assists residents, clients, or members with activities of daily living (ADLs) such as:
   1. Bathing;
   2. Eating; which includes set-up, serving and collecting food plates and utensils,
   3. Feeding residents who require assistance;
   4. Dressing;
   5. Oral hygiene; and
   6. Grooming as needed.
3. Assistance with toileting and provides appropriate peri care for incontinent residents, clients, or members.
4. Assists residents, clients, or members to turn, reposition, and transfer, which may be performed by the CNA without assistance, with a two person assistance, or by using a lift machine in accordance with the care plan.
5. Transports residents, clients, or members with ambulation to and from their room to appropriate areas for activities and meals.
6. Takes and records the resident’s, clients and/or members:
   1. ADLs
   2. Vitals (temperature, blood pressure, pulse, weight, and respiration rates)
   3. Intake and output of food and fluid
   4. Transfers (one or two person) and ambulation status
   5. Mood/behaviors when resistant to care
   6. Pain level assessment (if applicable)
7. Issues supplies as needed.
8. Provides companion services according to care plan.
9. Observes and reports to Licensed Staff all changes in condition (including significant changes), behavior or whereabouts that may require intervention.
10. Monitors and supervises residents, clients, and members as assigned.
11. Records all chargeable time provided by Arcadia Family of Companies (AFC).
12. Works in conjunction with the interdisciplinary team, licensed staff, team members, and other departments to ensure resident assessments are completed in accordance with corporate, federal, and state Regulations and safety guidelines. (Communities)
13. Adheres to assigned work schedules and accurately uses the time and attendance system, which may include using a computer, personal cell phone, or client’s home phone.
14. Participates in Continuous Quality Improvement and/or Quality Assurance Programs.
15. Interacts with residents, clients, and members in accordance with AFCs’ mission, core values, mindsets and guideposts.
16. Exercises caution and good safety judgment in uncertain conditions or situations, and report and/or correct hazards, potential or actual.
17. Ensures that all residents’, clients’, and members’ rights and privacy are protected and enforced, and establishes and maintains a close liaison with residents, clients, members’, and families.
18. Attends mandatory in-service programs.
19. Complies with annual employee health screening in accordance with policies and procedures.
20. Participates and assists during emergency drills and situations such as fire, fire alarms, hurricanes, and other possible emergencies as outlined in the Disaster Response Playbook and/or the AHHS disaster policy.
21. Other duties as assigned by the Arcadia Family of Companies.

Exclusive Essential Functions for CNA reporting to Day Care Center

1. Actively participates in the programs and activities of the Center.
2. Serves and collects food plates and utensils and pre-rinses and stores them properly for transfer to Arcadia.
3. Changes bed linens and washes and folds laundry.
4. Becomes familiar with the individualized service plan of assigned clients so as to carry out the Program’s services, goals, and objectives.
5. Monitors and supervises specific clients or activities as assigned.
6. Maintains Center’s facilities and equipment.
7. Facilitates and supports client participation in planned activities at or away from the Center by assistance with ambulation of clients.
8. Engages in positive interactions and promotes socialization during unscheduled periods.

Certificates, Licenses, and Registrations Current State of Hawaii Certified Nurse Aide certificate. Current First Aid and CPR certificate.

Computer Skills


Proficiency in using a computer, which may include using such software as Vision or Microsoft Office Suite (Excel, Outlook, PowerPoint, Word) and other applicable software applications.


Physical Demands While performing the duties of this job, the employee is regularly required to use hands to finger, handle, or feel; reach with hands and arms; and talk or hear. The employee frequently is required to stand, walk, sit, and taste or smell. The employee is occasionally required to climb or balance and stoop, kneel, crouch, or crawl; push and pull. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
