SUMMARY OF SOCIAL SERVICES DIRECTOR POSITION
At Progressive Care Center the Social Services Director assumes the authority, responsibility, and accountability and directs the overall operation of our facility’s Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures. The SSD also assures that the medically related emotional and social needs of the resident are met/maintained on an individual basis.
RESPONSIBILITIES
- Develop, implement, and maintain an ongoing quality assurance program for the social services department.
- Develop preliminary and comprehensive assessments of the social service needs of each resident and create a written plan of care for each resident that identifies social problems/needs and the goals to be accomplished for each problem/need identified.
- Participate in discharge planning, development and implementation of social care plans and resident assessments.
- Interview resident/families and involve the resident/family in planning social service programs and goals for the resident.
- Provide information to resident/families as to Medicare/Medicaid, and other financial assistance programs available to the resident.
- Develop and implement policies and procedures for the identification of medically related social and emotional needs of the resident.
GENERAL REQUIREMENTS
- Must be currently registered/licensed in applicable State. Must maintain an active license in good standing throughout employment.
- Three (3) years' experience in health-care social work preferred.
- Must be knowledgeable of the rules, regulations, and guidelines that govern nursing care facilities.
- Must possess the ability to make independent decisions when circumstances warrant such action.
- Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
- Must possess leadership ability and willingness to work harmoniously with and supervise other personnel.
