Provides pharmacy services and ensures the appropriate selection, dosing, administration, and dispensing of medication orders.

Responsibilities:

  • Conduct comprehensive appraisals of patients' health status by taking health histories, drug histories and performing appropriate physical examinations necessary to assess drug therapy.
  • Document relevant findings of a patient's health status in the patient's medical record.
  • Identify and address barriers to patient adherence.
  • Evaluate drug therapy through direct patient care involvement, with clinical assessment, subjective and objective findings relating to the patients' responses to drug therapy and documenting those findings in the electronic medical record.
  • Develop, document and execute appropriate therapeutic plans utilizing the treatments as per disease-specific policies.
  • Provide ongoing care for medical conditions as delineated in disease-specific standard operating procedures.
  • Provide patient education and medication information.
  • Evaluate and document patients' and caregivers ability to understand medication instructions and provide oral and written counseling on their medications.
  • Refer patients by consult to specialty clinics, order appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patients' drug therapy per disease-specific standard operating procedures.
  • Perform point-of-care testing if indicated in disease-specific standard operating procedures.
  • Recommend the appropriate use of medications including modifying or discontinuing drug therapy based upon a patient's formulary and/or per disease-specific standard operating procedures.
  • Analyze laboratory and diagnostic test data so as to modify drug therapy and dosing as necessary and in accordance with disease-specific standard operating procedures.
  • Identify and take specific corrective action for drug-induced problems.
  • Communicate relevant medication issues to other team members.
  • Other duties as assigned

Education:

  • Maintains an active, non-restricted Florida pharmacist license
  • Pharmacy consultant license, preferred; Must obtain consulting license within 6 months of hire date.
  • Board Certified Ambulatory Care Pharmacist; or If not board certified, must obtain certification within a year of hire or date of agreement, as applicable

Certifications:

  • Current State licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current State within 90 days of hire.  Board Certification preferred.
  • For individuals required to administer immunizations, current Basic Life Support (BLS) certification through American Heart Association (AHA) and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days of hire are required.  
    For individuals required to respond to adult or pediatric cardiopulmonary arrest events, current Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) Certification through American Heart Association (AHA), or certification obtained within one (1) year of hire, is required.

Complexity of Work:

  • Requires critical thinking skills, decisive judgment and the ability to work unsupervised or with minimal supervision.
  • Must be able to work in a stressful environment and take appropriate action.

Work Experience :

  • Completion of a post-graduate year 1 residency (PGY1), fellowship or 2 years of relevant post-graduate pharmacy experience required.
  • Post-graduate year 2 residency (PGY2) or relevant experience preferred for Clinical Pharmacists hired in a Specialized Pharmacy practice area (i.e. Critical Care, Oncology).
  • Current incumbents as of 6/1/2019 will be grandfathered for work experience requirement. 
  • At least five years of practice in a collaborative practice agreement or similar scope of practice.

Physical Requirements :

  • Typical physical demands include ability to perform the job while standing and walking for prolonged times, approximately 75% of shift and continuously for extended periods of time. 
  • Manual dexterity sufficient to perform computer and dispensing functions of the job; ability to lift up to 30 lbs.
  • Position often includes reaching, stretching, stooping, climbing steps and sitting.
  • Must have normal or corrected vision.
  • Must have ability to clearly communicate verbally by phone or in person

  • Follows standard precautions using personal protective equipment as required.
