Primary Function/General Purpose of Position

The Care Team Partner (CTP) works under the supervision of the RN to document in the patient’s electronic or written chart, which may be edited by the RN, with a final authentication and attestation by the RN, attesting that the CTP’s charting is accurate and complete. The CTP also assists in performing patient care duties, including, but not limited to, taking and recording vital signs, assisting with activities of daily living (ADLs), and responding to patient call bells. As a member of the care team, the CTP assists in practicing standard BSMH quality and safety measures to minimize the likelihood of patient safety and quality events, including, but not limited to pressure ulcers, falls,

Catheter-associated Urinary Tract Infections (CAUTI), central line bloodstream infection (CLABSI) DUKE protocols, etc.

Essential Job Functions

  • Completes documentation on the RN’s behalf, including, patient history, assessments, reevaluations, CTP admission and discharge notes, procedures, discussions with medical staff and family, and any medical decision making verbalized with the RN

  • Assists in the delivery of patient care by performing patient care tasks under the direction of a RN that are appropriate to age, mobility, acuity, and culture

  • Responds to patient call bells, obtains and records vital signs, activities of daily living (ADLs) which includes bathing, feeding, repositioning, intake, output, etc., indwelling urinary catheter care, blood glucometer checks, and placing patients on telemetry

  • Transports patient from one unit to the next or accompany the RN to transport patients to specialty or critical units of the hospital (depending on facility requirements)

  • Follows Centers for Disease Control and Prevention (CDC) isolation guidelines, demonstrate appropriate use of Personal protective equipment (PPE), perform handwashing according to policy, and enforce safety regulations

  • Exhibits a desire to learn and upgrade skills and perform other duties as assigned or directed to ensure to smooth operation of the department or unit

Licensing/Certification

BLS Basic Life Support – American Heart Association (required)

State Tested Nursing Assistant Licensure, or Certified Medical Scribe (preferred)

Education

High School/GED (required)

1 year of undergraduate coursework in the field of pre-nursing or other pre-health area of study (pre-med, pre-PA, pre-PT/OT, etc.) (preferred)

Work Experience

2 years of comparable experience as Patient Care Technician, State Tested Nursing Assistants (STNA), or medical scribe (preferred)

Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit.

Demonstrates knowledge of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his or her age, specific needs and to provide the care needed as described in departmental policies and procedures.
