SUMMARY Working under the supervision of the Director or designee, provides concrete services to individuals and their families and in accordance with ADAPT Community Network (ADAPT) policies, regulations and requirements of applicable regulatory agencies and the standards of the profession.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES The duties and responsibilities of the Intake Coordinator will include, but are not limited to the following:

  • Coordinates all aspects of Intake process. This includes but is not limited to scheduling appointments, explaining this process to parents, retrieving completed evaluations from consultants, preparing evaluation package for district, completing “awaiting approval” notices, receiving A1P’s and maintaining contact with families throughout this entire process.
  • Communicates with CPSE administrators and school personnel.
  • Conducts intakes and screenings to determine eligibility for services; as needed.
  • Participates in interdisciplinary team meetings and annual reviews and trainings as required.
  • Maintains contacts with government social service agencies, not-for-profit social service agencies and NYC Department of Education to access resources and/or advocates for services.
  • Keeps abreast of changes in field.
  • Safeguards information in relationship to individuals receiving services, including clinical, program and family information.
  • Performs other related duties as requested.

 

QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.   • Ability to read, write, speak and understand English
  • Good interpersonal skills are required in order to interact effectively with employees, co-workers, clients, families and outside contacts.
  • Good organizational skills and ability to meet deadlines
  • Proficiency in the use of a personal computer and knowledge of appropriate software
  • Due to sensitive nature of the position, must be able to maintain the strictest confidentiality.
  • Ability to work independently and handle multiple tasks.
  • Good organizational skills
  • Good oral and written communications skills
  • Demonstrate sound judgment and the capacity to respond to unusual situations
  • Ability to analyze problems and determine corrective actions.

 

EDUCATION and/or EXPERIENCE

  • Degree in Special Education or Social Work from an accredited program
  • One year of prior experience preferred as a Special Education Teacher, Social Worker, Case Manager or Early Intervention Coordinator
  • NYS Certification in Special Education or a NY State Licensed MSW or LCSW

 

Pay Rate: $40,000 - $71,000/annually
