Job Description - Admissions Coordinator

SERV, a well-established leader in behavioral healthcare in New Jersey, supports people with mental illness and/or intellectual/developmental disabilities as they achieve greater independence and life satisfaction through a wide range of services.

JOB SUMMARY:

The Admissions Coordinator oversees referral of consumers to SERV Achievement Centers Residential and Day Habilitation programs and acts as the primary liaison between the consumer, Access to Care, referral source , the insurance payer, billing department, and the program. The Admissions Coordinator will ensure that the insurer has preauthorized all consumers/applicants served, that authorization status has been appropriately documented, reviews completion of documents received and forwards in a timely manner to coordinators, directors, clinicians and staff for admissions review process. The Admissions Coordinator will serve as the agency liaison to the families and may provide case management services throughout the referral and admissions process through the first 30 days of their stay. The Admissions Coordinator will cooperate with the licensee and Department of Human Services staff in any inspection, inquiry or investigation.  

ESSENTIAL DUTIES & RESPONSIBILITIES:

  • Understand and follow SERV BHS’s Compliance and Ethics Program, its Employee Handbook, policies, procedures and rules, its mission, vision, and Core Values by exhibiting the following behaviors: excellence and competence, collaboration, innovation, respect, commitment to our community, accountability and ownership
  • Actively promotes admissions to the organization to build/maintain census; initiates and follows through on each step of the Admission process
  • Reviews pre-admission information for all referrals for Residential Services, Day Habilitation Programs; verifies eligibility/medical insurance (Medicaid and/or Medicare) prior to intake
  • Communicates routinely with referral sources, including Support Coordinators, DDD Offices, E-BLASTS, the Department of Children and Families (DCF), and other referral sources to recruit, clarify expectations and maintain collaborative relationships
  • Supports families and providers throughout the Admissions process
  • Ensures eligibility to care, including FFS and Tier appropriateness for IDD referrals
  • Assesses potential IDD referrals
  • Works closely with the Admissions and Achievement Directors/Coordinators to assist as needed and ensure referral entry/acceptance/denials
  • Works with Residential and/or Day Habilitation teams to identify appropriateness of placement and prevent any barriers to admission
  • Educates consumers as to their financial responsibility/ representative payee status
  • Collaborates with Support Coordination to secure Day Program services and/or transportation services in place by necessary timelines
  • Works with Behavior Support Services to transition appropriate cases into Residential /Day Habilitation Services
  • Generates progress and status reports as needed; maintains accurate data for average weekly census
  • Cross-train in other ATC department roles for backup/coverage
  • Additional duties as assigned or as needed to support departmental or organizational goals.

EDUCATION, KNOWLEDGE, SKILL & ABILITY:

  • H.S. Diploma minimum, bachelor’s degree in related field preferred.
  • Experience in residential and/or day habilitation
  • Case management experience
  • 3-5 years’ experience in Behavioral Health working with Mental Health and or IDD population strongly preferred.
  • Experience with electronic health record systems highly desired
  • Valid driver’s license in state of residence, clean driving record and reliable transportation for local travel.
