Responsibilities:
The Community Outreach Specialist is responsible for reaching out to eligible patients with the goal of enrolling them into HarmonyCares services. This will include an in-depth explanation of services, addressing a variety of inquiries, gathering feedback, and scheduling new patient appointments.
Essential Duties and Responsibilities
- Identify innovative strategies to enroll patients into HarmonyCares
- Develop relationships with Internal Medicine and Family Practices, Community Specialists, local hospitals, home health agencies, skilled nursing facilities, and clinics to drive engagement of eligible patients
- Establish a cadence of meeting with community partners to discuss HarmonyCares services, already engaged patients/families and eligible patients
- Address questions quickly while providing responses and meeting the needs of the patient and HarmonyCares internal team members
- Develop working relationships with practice Managers, PCPs, and key provider stakeholders to build formal business relationships
- Conduct telephonic outreach to eligible patients or families to enroll them in the HarmonyCares program
- Coordinating and/or facilitating enrollment and onboarding of new patients by assisting with patient consents, appointment set-up, and visit reminder notes
- Leverage reporting and dashboards to prioritize telephonic and field-based outreach to members
- Meet or exceed established monthly engagement targets
- Other duties as assigned
Qualifications:
Required Knowledge, Skills and Experience
- High school diploma
- 2+ years of related experience in sales, direct marketing, or customer service in a medical/healthcare industry
- Proficient in Microsoft Excel, Word, and PowerPoint
- Access to reliable transportation
- Non remote employees must maintain a valid driver's license and good driving record
- Ability to walk patients and families through enrollment
- Ability to communicate in an empathetic, compassionate, and professional demeanor at all times
- Ability to problem solve with demonstrated capacity to embrace complex problems and arrive at effective solutions in a timely manner
- Ability to analyze problems and resolve issues in a rapid, direct and complete manner
Preferred Knowledge, Skills and Experience
- Associates degree
- Provider engagement experience
- Prior Healthcare sales experience
- Experience with CRM platform
- Working knowledge of healthcare industry
- Proficient in PowerPoint and Excel
- Dynamic personality and a knack for developing good interpersonal relationships
- Comfort with scientific terminology
- Ability to seamlessly move through various medical environments, such as hospitals, doctors' offices and pharmacies
- Familiarity with several types of providers (i.e., Physicians, Physician Assistants, Nurse Practitioners)
