Under the general supervision of the Division Chief – EMS, the Community Paramedic provides follow-up assessments and treatment, in partnership with other health professionals. To provide access to care, while lessening the burden on the 911 system. These services include but are not limited to the post-COVID impacts of disease management and education, prevention and wellness, mental health, and home safety evaluations.

This is a Full-Time limited duration (grant funded) position that is expected to last no longer than 2 years. Limited duration positions are eligible for benefits. 

The functions listed below are characteristic of the type and level of work associated with this position. They are not all-inclusive. Individual may perform some or all, as well as other similar work.

  • Performs essential functions of a paramedic
  • Examines, screens, treats and coordinates health services for patients
  • Performs initial in-home assessments and on-going home visits as appropriate
  • Conducts follow-up visits to promote continuity of care for substance abuse disorders, mental health, elderly care and other conditions.
  • Utilizes record management systems to document treatment, actions and response data for coordination with the Fire District, physicians and case manager.
  • Performs prevention and education duties to the public, addressing various social determinates of health.
  • Administers patient care consistent with EMS protocols and Fire District policies
  • Coordinates appointments and follow-up with case workers, physicians and other health care professionals
  • Completes other required reports and documentation.
  • Attends meetings as requested.
  • Performs other duties as assigned.

Minimum Qualifications
  • High School Diploma, GED, or College Degree
  • Valid driver’s license and acceptable driving record
  • One year of experience in the healthcare field 
  • Paramedic License or NREMT License at the time of application

Preferred Qualifications
  • Trained and/or experience as a Community Health Worker
  • Additional training in Community Paramedicine
  • Knowledge, experience and understanding related to current EMS practices, including current standards of care for cardiac, medical, trauma, obstetric, pediatric and geriatric emergencies
  • Ability to remain calm in emergency situations
  • Ability to read and understand, and communicate effectively in English, both orally and in writing
  • Bilingual 
  • AHA Advanced Cardiac Life Support (ACLS) certification or equivalent
  • AHA Pediatric Advanced Life Support (PALS) certification or equivalent
  • Prehospital Trauma Life Support (PHTLS) certification or equivalent
