Job Description - Counselor

Main Function:

  • The Counselor is responsible for assessing, counseling, intervening, implementing and educating the patient and family/significant others to facilitate movement throughout the continuum of care.
  • Makes psychosocial assessments appropriate to patient’s age and diagnoses.
  • Appropriately prioritizes cases according to assessed needs
  • Written assessments completed within 24 hours of assessment
  • Documented assessment demonstrates understanding of patient’s functional status and psychosocial situation
  • Assessments are completed based on information obtained from patients, families/significant others, staff and interdisciplinary rounds as needed
  • Assessment prepares for the possibility of changes in the patient’s condition/status
  • Provides supportive counseling to patients and families
  • Coordinates discharge planning services.

 

Full Time * 8-hours/Day shifts 
Rotating weekends and holidays

 

  • Bachelor’s Degree required in Counseling, Social Work or related field
  • 0 – 2 years required
  • CADC/LCADC must be obtained within 3 years of date of hire.
