Key Responsibilities:
- Administer medications and treatments as prescribed by physicians.
- Monitor and record patient vital signs medical history and progress.
- Assist with daily living activities such as bathing dressing and mobility.
- Provide wound care and manage catheters or feeding tubes as needed.
- Collaborate with registered nurses physicians and other healthcare team members to develop and implement patient care plans.
- Educate patients and their families on health maintenance and disease prevention.
- Maintain accurate and uptodate patient records.
- Ensure a safe and supportive environment for all patients.
Qualifications:
- Valid and active LPN license in the state of Pennsylvania.
- Minimum of 1year experience working in a healthcare setting.
- CPR/BLS certification required.
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a team.

- Excellent organizational and time management skills.
