Annual Salary:

Y15B - $59,437.40/ Per Year + Full State Benefits Package

Minimum Qualifications:

• Possession of a valid permanent Oklahoma license as a registered nurse as approved by the Oklahoma Board of Nursing or Registered Nurse Licensure Compact (eNLC)
• Three years of professional nursing experience

Basic Purpose of Position

DDS Health Care Management Nurses are required to perform annual or more frequently indicated reviews for Community and Homeward Bound Waiver service recipients.  They also complete PASRRs, mortality reviews upon the death of any waiver recipient, and other activities to ensure teams have the information and resources necessary to meet the healthcare needs of recipients.

• Identifies assigned service recipients' health concerns and consults with the case manager to assist in facilitating the identification of health risks with recommendations before the annual  Individual Planning (IP) meeting on the following task below,  90% of the time. 
• Completes a Health Review and updates the Medical Event, Diagnosis, Allergy, Medication, and Provider section in CCM. 
• Analyzes data from documentation such as home records, CCM, DDS-5, DDS-6, Incident Reports, IP, agency, and electronic communications to assist case managers in identifying health risks/concerns of service recipients. 
• Consult and submit a written Health Review to the case manager with recommendations to develop the Health Section of the IP. 
• Develop a Nursing Service Support Plan as indicated. 
• Emergency PASRR assessments are submitted to the state office within 5 calendar days.
• Routine and Reconsideration PASRR assessments are submitted to the state office within 7 working days.
• Initial 24-hour mortality Report completed within 24 hours of notification of service recipient’s death and submitted to the Nurse Manager and state office
• Mortality Reviewer completes Final Mortality Report and facilitates Mortality Review Committee meeting within 30 days of receiving records.
• Service recipient transitions and transfers.
