The Licensed Practical Nurse accepts accountability for provision of care, as part of the RN led team, in accordance with the current policies and procedures. • Education : High School Diploma or GED, required. Graduate of accredited Practical Nursing Program, required. 
• Experience : One year of relevant experience, required. 
• Licensure/Certification : Appropriate state LPN license, required.
