ResponsibilitiesIn the role of the RN Case Manager, the Registered Nurse is responsible for coordinating the care of a population of patients throughout their hospitalization with the goal of appropriate resource utilization and preparation for a timely discharge plan that meets individual patient care needs. The RN Case Manager will collaborate closely with other disciplines including social workers, providers, utilization review, and ancillary departments. Through this collaboration, and the use of care guidelines, the RN Case Manager helps to ensure progression along clinical pathways and is involved in creating a safe transition into post-acute settings. The RN Case Manager represents Virginia Mason with professionalism when interacting with community agencies and facilities.

QualificationsRequired:
- Bachelors of Science in Nursing
- Minimum of 3 years of experience in care coordination, care management, discharge planning, or relevant/related experience - Current Washington State Registered Nurse License
Preferred: - Master's degree in Nursing
