Registered Nurse

Duties:

• This RN will be responsible for visiting patients in their home conduct patient visits to ensure that care plans are being followed and updated as necessary.
• Develop and maintain patient care plans according to agency policies and procedures.
• Communicate with patients, their families, and other healthcare professionals to coordinate care and ensure patient satisfaction.
• Conduct regular reviews of patient charts to ensure that all required documentation is accurate and up-to-date.
• Collaborate with other healthcare professionals to ensure that patients receive the highest level of care.
• Work with agency leadership to develop and implement quality improvement initiatives.
• Ensure that all patient information is kept confidential and in compliance with HIPAA regulations.
