Role Description

This position on the population health team will lead our population health pharmacy team as a direct manager to a team of clinical pharmacists. The Manager, Clinical Pharmacy will serve as the owner for the clinical pharmacist portion of Oak Street’s transitional care Medication Reconciliation Post Discharge (MRP) and support the relationship between Oak Street and our trusted pharmacy partners. This position will spend approximately 33% of their time in direct patient care and 67% performing administrative and strategic activities. He or she will serve as a mentor, coach, and advisor to pharmacists at Oak Street Health and participate in pharmacy department strategic initiatives.

Responsibilities

  • Directly manage team of Ambulatory Care Clinical Pharmacists and serve as cohort leader for CVS Pharmacy clinical pharmacists

  • Develop, implement, and manage clinical pharmacy programs, such as chronic condition management and comprehensive medication reviews

  • Develop collaborative practice agreements to support clinical pharmacy activities across multiple states

  • Proactively use data and analysis to track, benchmark, and communicate the outcomes of clinical pharmacy interventions

  • Review Discharge Summaries and engage patients, caregivers, providers, and/or outside pharmacies to complete medication reconciliation post discharge (MRP)

  • Evaluate post discharge medication orders, and review medication profiles to prevent drug interactions, drug-food interactions, drug-laboratory interactions, drug duplications, and allergic reactions to medications

  • Ensure the appropriateness of drug therapy based upon patient's diagnosis, the correctness of the dosing of medications based upon the patient specific parameters, and promotes selection of the most cost effective medications that attain the desired therapeutic outcomes

  • Contribute to patient care plans in concert with the patient’s primary care physician or managing medical team.

  • Issue recommendation to patient’s managing medical team regarding therapies or alternative approaches based on interpretation of relevant medical literature and clinical experience.

  • Make verbal or written recommendations in a clear, succinct and appropriate manner.

  • Consider age, weight and organ function (e.g. renal, hepatic) when evaluating patients and patient’s therapies when making recommendations.

  • Interview patients to determine medical history, previous medication condition and response to therapy.

  • Interpret laboratory and other patient-specific data to develop treatment plans and to monitor response to therapy.

  • Apply pharmacokinetic principles in order to achieve efficacious, safe and economical pharmacotherapeutic regimens.

  • Respond to therapeutic queries posed by physicians and other healthcare providers.

  • Identify and correct drug-related problems such as complications resulting from drug therapy (ineffective therapy, allergic reactions and adverse drug reactions).

  • Establish procedures for detecting significant drug-drug, drug-laboratory and drug-food interactions. Make recommendations to avoid such interactions.

  • Ensure a continuum of care by cooperating with other health professionals and outside health organizations.

  • Counsel patients and/or caregiver as to the proper administration, potential adverse reactions, storage of medications, etc.

  • Participate in performance improvement activities including problem identification, data collection, solution selection, implementation and evaluation; assists in the care and maintenance of departmental equipment and supplies and other duties as appropriate

  • Cascade both organizational information to pharmacy team members as well as escalate care team concerns and ideas to the rest of the organization.

  • Help coordinate and lead the onboarding of new pharmacists

  • Create and maintain a culture of coaching up and growth of pharmacists

  • Listen to feedback from team members and address concerns with constructive problem-solving.

  • Conduct performance reviews and discuss team member goals and help develop plans to achieve these goals.

What we’re looking for

We’re looking for motivated, experienced clinical pharmacist with:

  • This is a remote position and the candidate can be located anywhere but must be licensed as a pharmacist in good standing in the state they reside

  • 3 or more years experience in medication therapy management, Medicare STARS or HEDIS quality measures, managed care pharmacy, or population health 

  • Experience in clinical leadership roles, leading and coaching providers to be the best they can be for their patients and their colleagues.

  • Excellent customer service skills and phone etiquette

  • Strong working knowledge of MS Office and Google documents/spreadsheets

  • Ability to synthesize and analyze large amounts of data to make sound business decisions

  • Board certification (BCPS, BCACP, BCGP, etc.) and other professional degrees (e.g., M.B.A., M.P.H.) welcomed but not required.
