RN Case Manager

Location: Baltimore, MD

Department: Case Management

Shift Timings: 08:00 - 16:30 Hours, Monday to Friday (5 Days/8 Hours)

Per Week: 40 Hours

Summary: We are currently seeking a dedicated and compassionate RN Case Manager to join our Case Management team. The ideal candidate will be responsible for creating comprehensive care plans, monitoring patient care, advocating for resources, and maintaining effective communication with healthcare providers to ensure the implementation of optimal health plans.

Roles & Responsibilities:

• Create plans for care across the continuum, integrating patient/family preferences and values.
• Monitor patient care through assessments, evaluations, and/or patient records.
• Advocate for resources and removal of barriers to ensure optimal patient care.
• Maintain ongoing dialogue with supervisors and other health providers to ensure the effective implementation of health plans.
• Act as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care.

Qualifications:

• Current Registered Nurse (RN) license in Your State.
• Bachelor's degree in Nursing preferred.
• Minimum of 1-3 years of experience in case management or related healthcare field.
• Strong communication, organizational, and critical thinking skills.
• Ability to work collaboratively in a team environment.
• Proficiency in electronic medical records systems preferred.
