Provide prompt and compassionate nursing care to patients in their homes during on-call hours, including evenings, nights, weekends, and holidays.
Respond to urgent patient calls and triage situations to determine the appropriate level of care.
Conduct thorough assessments of patients' conditions during on-call visits and implement necessary interventions.
Administer medications, treatments, and procedures as needed, ensuring patient safety and comfort.
Communicate with primary care physicians, hospice medical directors, and other healthcare providers to coordinate and update care plans based on patient needs.
Offer emotional support and education to patients and their families, addressing concerns and providing guidance on symptom management.
Document all patient interactions, assessments, and interventions accurately and timely in the electronic health record system.
Collaborate with the interdisciplinary team to ensure continuity of care and optimal patient outcomes.
Participate in regular on-call rotations as part of the nursing team.

Registered Nurse (RN) license in good standing.
Previous experience in home health care or hospice care is preferred.
Strong clinical skills and knowledge of evidence-based practices in nursing.
Excellent communication and interpersonal skills.
Compassionate and empathetic attitude towards patients and their families.
Ability to work independently and as part of a collaborative team.
Flexibility to respond to patient needs during on-call hours.
