Envision Physician Services is looking for Acute Care Nurse Practitioners and Physician Assistants to join our growing team at HCA Houston Healthcare Medical Center in Houston, TX!

ICU experience required - *cannot consider new grads for this role* Work approximately 15 shifts per month 12 hour shifts Work alongside an experienced team of Critical Care physicians Flexible work schedule allowing for blocked time off Employment status eligible for benefits Using evidence-based processes through a multidisciplinary approach 12 medical and 12 surgical beds Intensivist support ECMO, mechanical cardiac support and complex CV surgery programs

• MUST BE FULLY VACCINATED VAX CARD ON FILE
• House Calls hours of operation are Monday-Sunday 8am to 8pm. 
• Weekend hours are completely fine. Client is flexible part time hours available. 
• Training, equipment and supplies provided by Client

Position Summary: House Calls Nurse Practitioners conduct in-home health assessments on enrolled Medicare Advantage members. Nurse Practitioner will complete visits on members within a geographic area of responsibility.  The nature of the visit can vary and could include performing an annual health assessment, a post discharge visit, or visiting more complex members more than once per year.

Qualifications

Required Education and Experience:

• NP license and national certification (ANCC or AANP, or the ability to obtain national certification and/or NP license in state of assignment) active, unencumbered licensure free any open/unresolved disciplinary actions including probation or restrictions against privilege to practice
• Minimum of 6 months clinical experience in their highest level of education, clinical setting preferred.
• Family, Geriatrics, or Adult certification required
• Experience in gerontology, cardiology, internal medicine, or endocrinology a plus.
• Home Health care or home visit experience is a plus.
